- Evaluates the Patient’s nutritional diet during the first initial home visit
- Develops and implement a nutritional plan based on an assessment of nutritional needs. This plan is included in the Patient’s Plan of Care
- Plan of care will be created by a Registered Dietitian to adapt to the Patient’s lifestyle
